Models of Practice

South Madison Health and Family Center - Harambee, Madison, WI

With support from neighborhood and community agencies, the South Madison Health and Family Center-Harambee (SMHFC-H) was created to provide community-based, culturally sensitive, comprehensive and coordinated health and education services to serve, promote and support healthy independent families and individuals. This umbrella agency for human and health services (housed at one site), includes Head Start, Planned Parenthood, South Branch of Madison Public Library, Early Childhood Family Enhancement Center, Madison Community Health Center, Madison Dept. of Public Health, and Dane County Dept. of Public Health.

The goals of the Harambee Center are to:

  • Foster a partnership among neighborhood residents, service providers and the community to achieve common goals.
  • Bring together community-based, culturally sensitive and coordinated services in an accessible, welcoming environment.
  • Meet the health and education needs of parents and children in a comprehensive participant-focused way.
  • Develop a partnership that responds to community needs by including neighborhood residents in planning and governance.
  • Provide transportation services, translation services and advocacy services that will improve access.
  • Provide a place where providers and participants learn from each other.

Partnership agreements commit all agencies to meet the needs and build on the strengths of the community and individual customers and families.

The Center's motto says it all - "A place to grow, learn, stay healthy and be strong."
